The Spaulding Classification for the CRCST Exam
Updated October 27, 2026
The Spaulding classification decides how much processing a reusable patient care item needs: sterilization, high-level disinfection or low-level disinfection. It’s based on how the item contacts the patient, and it’s a core idea in sterile processing and on the CRCST exam. Here’s how it works, followed by free practice questions.
Always follow the device’s instructions for use (IFU) and your department’s policies, which take priority.
The three categories
| Category | Contacts | Minimum processing | Examples |
|---|---|---|---|
| Critical | Sterile tissue or the vascular system | Sterilization | Surgical instruments, implants, needles |
| Semi-critical | Mucous membranes or non-intact skin | High-level disinfection (sterilization preferred when possible) | Flexible endoscopes, respiratory therapy equipment, some probes |
| Non-critical | Intact skin only | Low-level disinfection | Blood pressure cuffs, stethoscopes, bedpans |
Key points
- Cleaning always comes first. No disinfection or sterilization works on an item that isn’t clean, because soil shields microorganisms.
- Sterilization destroys all microorganisms, including bacterial spores.
- High-level disinfection destroys all microorganisms except large numbers of bacterial spores.
- Low-level disinfection kills most vegetative bacteria and some viruses and fungi.
Free Spaulding classification practice questions
-
A surgical instrument that enters sterile tissue is classified as:Answer: AItems that enter sterile tissue or the vascular system are critical and must be sterilized.
-
What is the minimum level of processing for a semi-critical item?Answer: CSemi-critical items contact mucous membranes or non-intact skin and need at least high-level disinfection. Sterilization is preferred when the item can tolerate it.
-
A blood pressure cuff that touches only intact skin is classified as:Answer: BItems that contact only intact skin are non-critical and need low-level disinfection.
-
What must happen before any item is disinfected or sterilized?Answer: BCleaning comes first. Soil and bioburden can shield microorganisms and stop disinfectants and sterilants from working.
-
A flexible endoscope that contacts mucous membranes is usually classified as:Answer: DA flexible endoscope that contacts mucous membranes is semi-critical, so it needs at least high-level disinfection, following its instructions for use.
-
Which process destroys all forms of microbial life, including bacterial spores?Answer: ASterilization destroys all microorganisms, including spores. High-level disinfection kills all microorganisms except large numbers of bacterial spores.
